In order for your child to be fully enrolled, the school needs the following documents:

 Proof of immunization: (Blue) Health Assessment Record

 Health Information (White)

 Copy of Birth Certificate

 Copy of Social Security Card

 Evidence of Residency (utility bill, lease, forwarded mail)

 Proof of Guardianship/Parental Custody, if applicable

 If not a citizen, Proof of immigration status must be provided

Copy of school records.  We will receive these after you have signed a green 'Release of Records' authorization form and this is forwarded from the sending school, if applicable.
